Description
The Indiana Army National Guard requires a dental office to provide extensive treatment for soldiers as needed, including root canal therapy, crowns, surgical extractions of exposed-root, impacted, and erupted teeth, core build-ups, and associated procedures. Services must be performed at the contractor’s own dental office during normal business hours, with appointments available within 14 calendar days of a treatment request. The office must accept Concordia and Blue Cross Blue Shield insurance and bill that insurance before using government funds. The contractor must provide treatment estimates based on SF 603 requests, obtain approval for any treatment outside the approved plan, submit monthly treatment-and-cost trackers and invoices, and provide a signed DD Form 2813 with each soldier’s resulting dental readiness code. The purchase order is unpriced and not to exceed $100,000, with a one-year period of performance from September 21, 2026, through September 20, 2027. Responses should include the office location, relevant government-contracting experience, insurance acceptance, ability to perform the PWS tasks, and a retail or fee sheet.
