Description
The Grand Traverse Band of Ottawa and Chippewa Indians is requesting proposals from dental hygienist providers to deliver as-needed hygienist services for patients of all age groups at the Grand Traverse Band Dental Clinic. The contract provides for up to three days per week, Monday through Wednesday; the hygienist will be paid for patient cancellations and Tribal government closure days, excluding holidays. The provider must be a licensed hygienist in Michigan, hold CPR certification, maintain liability insurance, and pass a background check conducted by the Grand Traverse Band. The proposal must include a cover letter, resume documenting qualifications and past experience, and a bid stating the expected hourly rate. Proposals will be evaluated based on qualifications, past experience, and cost, with a possible follow-up interview. Native American/Alaskan Indian preference applies, and bidders must submit proof of Native American Indian status with their bid.
