Clarkston School District is issuing a Request for Proposal (RFP) for a health care organization to provide preventative and primary health care services, including physical health, mental and behavioral health care, and care coordination, through a School Based Health Center (SBHC) located on a school campus. The SBHC aims to remove barriers to health care for underserved students, reduce absenteeism, and support academic success. The selected organization will enter into a rental agreement with the district and must meet minimum hours of operation: 10 hours/week for physical care, 10 hours/week for mental and behavioral health care, and 15 hours/week for care coordination. The district has secured $750,000 in capital funds to build the SBHC and is working to secure up to $250,000 in startup costs through a DOH grant. Proposals must address core competencies including access, student focus, high-quality care, collaborative school partnerships, school wellness, systems coordination, and sustainability. The application packet should not exceed 50 pages and must include a cover letter, organization references, organization information, team information, technical qualifications, and a sustainability plan.
