Description
Trumbull County Department of Job and Family Services is soliciting one or more providers to deliver Title XX services to eligible, low-income elderly and disabled adults. Service categories include Home Health Aide, Homemaker, Chore, and Adult Protective Services, including assessment, guardianship, and conservatorship support. Services must promote self-support, self-care, independence, and safe living, and must comply with applicable federal, state, county, and program requirements. The solicitation anticipates funding of up to approximately $75,000 for all service categories combined, subject to available funds and demand; individual awards may cover one or more categories and may be divided among multiple providers. Bidders must describe their qualifications, service-assessment and delivery methods, outcome and satisfaction measures, administrative capabilities, proposed costs, and any subcontractor involvement. Required supporting materials include organizational and legal-status documentation, insurance records, financial and tax information where applicable, disclosures and attestations, and active SAM registration information. The submission must include an original signed bid and one USB flash drive containing the required PDF documents.
