Description
Butte County seeks a qualified organization to expand home-delivered meal nutrition services to 100 unique, eligible adults age 60 and older, prioritizing underserved communities. The provider will propose its service area, delivery routes, and beneficiary reach, and supply nutritionally appropriate meals that meet applicable dietary guidelines while accommodating dietary restrictions where feasible. The work includes outreach and referral coordination, equitable access and compliance with nondiscrimination and Community Development Block Grant requirements, beneficiary and program data collection and reporting, and support for grant administration, monitoring, and closeout. Proposals must describe the provider’s experience, staffing and operational resources, approach, itemized costs, and draft scope of work and timeline. The stated funding ceiling is $280,374 for a 33-month term.
