Description
Los Alamos County is seeking a New Mexico-licensed medical provider to operate Health Commons services, including confidential family planning for teens and reproductive-age adults, STI screening and treatment, partner services, primary care, vaccinations, physical examinations, acute and chronic condition treatment, health promotion, screenings, referrals, and behavioral health services addressing mental health and substance use. Services must be available at least three days per week, accommodate low-income and uninsured patients through sliding-fee arrangements, support confidential no-cost teen services, and accept authorized private insurance payments. The provider must maintain required medical, pharmacy, business, and professional licenses; provide qualified staffing and proposed operating hours; coordinate referrals and care for at-risk youth and families; use the Unite Us referral platform; protect patient privacy and comply with HIPAA and applicable laws; and submit quarterly progress reports and annual financial information. The proposal must include qualifications, relevant experience, a detailed service approach, staffing plan, budget for all potential contract years, and the required County forms and supporting materials.
