Description
The City of Minneapolis Health Department is seeking qualified nonprofit, LLC, or for-profit organizations to implement or expand community-centered doula and care coordination services for pregnant and postpartum Minneapolis residents. Services must be community-based, culturally grounded, strengths-based, trauma-informed, and focused on birth equity and birth justice. Eligible participants must reside in Minneapolis, be pregnant or within 12 months postpartum, and meet specified low-income and/or high-risk criteria. Services may include prenatal, labor and delivery, and postpartum doula support; needs assessments; individualized care plans; referrals and warm handoffs to medical, behavioral health, housing, food, transportation, utility, parenting, legal, and other community resources; and documentation of referral outcomes. Clinical medical services and home health care are not eligible. The Health Department anticipates three to four awards totaling $230,000, with individual awards anticipated to range from $50,000 to $75,000 annually. Applicants must submit a project narrative of no more than 10 pages, a completed proposal cover sheet, a budget and budget narrative, and company financial information, including bankruptcy filings from the previous seven years and a current ratio or recent audit or financial statement. Awardees will be reimbursed for actual documented costs and must provide quarterly progress reports and invoices, with a final invoice and project summary or presentation. Administrative costs are capped at 10% of direct costs.
