Description
The Indian Health Service is seeking a contractor to provide a comprehensive Therapeutic Shoe Program for eligible American Indian and Alaska Native patients receiving care through the Lawton Service Unit. The scope includes diabetic and therapeutic footwear, custom or prefabricated inserts, diabetic socks, orthotics, related diabetic foot supplies, and associated modifications. Required services include comprehensive diabetic foot assessments, measurements, gait and clinical evaluations, fittings, dispensing, adjustments, replacements, patient education, and follow-up care. The contractor must coordinate with the Lawton Service Unit, maintain sufficient inventory and qualified credentialed personnel, provide onsite clinics and deliveries as directed, protect patient information, submit monthly performance reports and detailed invoices, and maintain applicable accreditation, licenses, insurance, and professional credentials. The base period is 12 months from award with three additional 12-month option periods.
