Description
The City of New Haven and New Haven Public Schools Food Service is soliciting shelf-stable cheese for school lunch, community-center, and emergency food programs. The requested products include 1-ounce vintage, ranch, jalapeño, and cheddar cheese sticks or planks, or equal products, with quantities of 20,000 portions per line item. Products must be individually wrapped, tamper-evident, clearly labeled, food-grade, room-temperature stable, and have at least 12 months of shelf life from delivery. They must comply with applicable FDA and USDA requirements and be free from artificial preservatives, colors, flavors, foreign materials, and contaminants. The awarded vendor must provide order fulfillment within 10 business days, order confirmation within 24 hours, customer support during regular business hours, quality-assurance documentation upon request, recall and return support, and delivery to multiple New Haven schools, community centers, and municipal facilities. Proposals require separate technical and price submissions, with pricing including a total lump-sum price and detailed cost breakdown. The solicitation also requests a complete item list, qualifications information, three similar-product references, required affidavits and certifications, and a current workforce demographic form.
