Description
The Texas Workforce Commission is accepting applications from qualified providers to deliver Diabetes Self-Management Education Services to Vocational Rehabilitation customers throughout Texas. Services include individualized assessment, diabetes skills training, adaptive equipment education, behavior-change support, and post-training assessment for customers who are blind or visually impaired, newly diagnosed, unable to maintain employment because of diabetes, or unable to benefit from standard community-based programs. Providers must meet specified diabetes educator qualifications, maintain required continuing education, complete background checks for staff who interact with customers, and comply with the current Vocational Rehabilitation Standards for Providers Manual. Initial assessment and skills training are generally provided in person, while certain services may be delivered remotely with prior approval. The solicitation provides fee rates of $117 per hour for assessment, individualized training, and post-training assessment, and $60 per hour for group training. TWC makes no guarantee of volume, usage, or compensation. Contracts will run for five years from execution, and applications may be submitted throughout the open enrollment period until the stated closing date, subject to earlier closure.
