Description
This Sources Sought notice seeks information from qualified businesses capable of providing on-site basic medical services at Jacobs Creek Job Corps Center. Services are intended for students generally ages 16 to 24 and include examinations, assessments, diagnosis and treatment of acute and chronic conditions, prescriptions and immunizations, health screenings, first aid and CPR, referrals, health promotion and education, medical record documentation, and coordination with center personnel and other health and wellness programs. The draft statement of work requires licensed medical professionals, such as a physician, nurse practitioner, or physician assistant, to provide services on center generally one or two weekdays per week during mutually agreed hours, with a reasonable example schedule of 8:00 a.m. to 5:00 p.m. The provider must maintain applicable licenses, certifications, and insurance; comply with HIPAA, infection-control, federal, state, and Job Corps requirements; and support reporting, quality-improvement activities, assessments, and student health-record management. Responses are for market research only; no proposals are being requested and no contract will be awarded from this notice. Respondents must provide at least a capabilities statement and their SAM Unique Entity Identifier.
