Description
The Delaware Department of Health and Social Services seeks a vendor to operate the Delaware Quitline, a toll-free tobacco cessation hotline providing telephone and in-person counseling. Services include counseling for tobacco users, access to trained health-care professionals, online cessation support, and a pharmaceutical cessation-aid program providing eligible participants with FDA-approved prescription cessation aids and non-prescription nicotine-replacement products such as patches, gum, and lozenges for up to eight weeks. The contractor will support Delaware's tobacco-prevention and cessation objectives, including increasing quit attempts, reducing exposure to tobacco and emerging products, and providing related participant services and program support. The submission must include a signed transmittal letter, proposal or executive summary, completed vendor forms in Appendix C, required responses to the minimum mandatory requirements, and any applicable inclusion-plan or confidentiality materials. Appendix D contains templates used in the contracting process; exceptions must be identified in the exceptions form.
