Description
Clark County, Ohio, is soliciting competitive sealed proposals for consulting services related to employee benefits. Respondents must provide the full name and address of each proposing person or company and submit one original proposal and three copies in a sealed, clearly marked envelope. The proposal package must include completed responses to Exhibit C, identified as the Request for Proposal Response for Near-Site Clinical Services, along with the Non-Discrimination Provision, Personal Property Tax Statement, Non-Collusion Bid Affidavit, and Affidavit of Compliance with Ohio Revised Code Section 3517.13. The successful bidder must later provide a current Ohio Bureau of Workers' Compensation certificate and a certificate of liability insurance meeting the County's requirements.
