Description
Valley Park School District is requesting proposals for administration and management of its Section 125 plan and for ancillary insurance products for district employees. Requested offerings include health and dependent-care flexible spending accounts, health savings accounts, employee voluntary life and accidental death and dismemberment insurance, employee voluntary universal life, spouse and child voluntary life, short- and long-term disability, hospital indemnity, and cancer coverage. Proposals must provide pricing for each requested product, describe any additional services, complete the attached questionnaire, and include a bid sheet. Respondents must explain their approach to Section 125 administration, enrollment, employee education, billing, coverage changes, claims handling, district costs and risks, portability, and points of contact. Three active professional references, preferably from other K-12 institutions, are also requested.
