Description
The West Virginia Department of Health, Bureau for Public Health, Office of Maternal, Child and Family Health, is soliciting one subrecipient grant proposal for a statewide Family Support and Engagement Initiative serving children and youth with special health care needs and the professionals who support them. The selected grantee will provide family information, education, technical assistance, peer support, referral follow-up, resource navigation, outreach, accessible online and written resources, family support events, and coordination with the state Children with Special Health Care Needs Program. The grantee must hire and maintain project staff, including a Project Manager and Parent Network Specialists; administer an online project resource center; support social media engagement; distribute welcome packets and program materials; participate in care coordination and advisory activities; support LEND-related capacity building; and submit monthly and annual performance reports. The grant period is October 1, 2026, through September 30, 2027, with anticipated annual funding of up to $500,000. Applicants must submit a proposal narrative, detailed line-item budget and budget narrative, Subrecipient Information Form DHHR Finance A-1000, and a Grantee Contact List through the West Virginia OASIS Vendor Self Services portal.
