Description
The Town of Nantucket, through Our Island Home Skilled Nursing Facility, is seeking a medical director to provide clinical oversight, physician leadership, resident-care policy guidance, quality-improvement support, infection-control guidance, regulatory and survey support, staff education, and related medical and clinical services. The scope requires at least one on-site visit and participation in designated committee meetings each month, with services generally limited to 18 hours per month unless additional hours are pre-approved. The contract term is January 1, 2027, through December 31, 2027, with options to renew for two additional one-year terms. Responses must include the required tax compliance and non-collusion forms, references for at least three similar customers served within the past three years, experience information, insurance confirmations and a current insurance certificate, an executed contract signature page, and pricing for the base year, two renewal years, and additional hourly services.
