Description
Springfield Public Schools is soliciting all labor and materials to replace the hot water holding tanks at Parkview High School. The response must describe the proposed services, company experience and workforce, organizational references, local-government experience, business and emergency-call procedures, local representative information, proposed start time, substantial- and final-completion schedules, and manufacturer accreditation from approved manufacturers. The project requires prevailing-wage compliance, construction-safety training for applicable on-site personnel, daily site cleanup, off-site disposal of demolition materials, and a minimum one-year craftsmanship warranty. Bidders must provide a 5% bid guaranty and payment and performance bonds equal to 100% of the contract sum. Required submission items include the Bid Form, Subcontractor Form, Bid Bond, E-Verify, DBE Form, Substitute W-9, Felony Conviction Notification Form, and Federal Work Authorization Program Affidavit, along with the requested response information and bid pricing. Insurance evidence is required, including general liability, automobile liability, workers' compensation, employers' liability, professional liability when applicable, riggers liability when applicable, and contractors pollution liability when applicable. A site visit must be scheduled through the District Representative and attended for the bid to be accepted.
