Description
The Department of Veterans Affairs, through Network Contracting Office 21, seeks a contractor to provide residential treatment services for eligible San Francisco VA Health Care System beneficiaries who are homeless, at imminent risk of homelessness, or have urgent mental health needs. Services must be provided at the contractor’s facility within San Francisco City and County and include room and board, meals, laundry access, 24-hour emergency staffing, therapeutic and rehabilitative services, group activities, case management, living-skills support, discharge planning, medication management, transportation assistance, and coordination with VA and community resources. The facility must maintain appropriate clinical, safety, sanitation, fire-protection, accessibility, medication-security, confidentiality, records-management, and pest-control practices. The solicitation requires a service plan, quality control plan, facility license, contingency plan, and three similar recent past-performance references. The Government intends to award a single-award firm-fixed-price indefinite-delivery, indefinite-quantity contract with a five-year ordering period, a guaranteed minimum of $500, and a maximum aggregate ordering value of $1,600,000, with a possible six-month service extension.
