Description
Valley Park School District is requesting proposals for administration and management of its Section 125 plan and provision of ancillary insurance products for district employees. Requested products include health and dependent-care flexible spending accounts, a health savings account, voluntary employee/spouse/child life insurance, accidental death and dismemberment coverage, short- and long-term disability, hospital indemnity, and cancer insurance. Proposals must provide pricing for each requested product, describe any additional services, complete the attached questionnaire, and include a bid sheet. Respondents must explain their approaches to Section 125 administration, employee enrollment and education, billing, coverage-change notifications, claims handling, portability, district costs and risks, and points of contact. Three active professional references, preferably from K–12 institutions, are also requested.
