Description

This solicitation is for the renovation of four structurally sound modular homes at the Kayenta Health Center in Kayenta, Arizona. The existing homes, built in 1978, require interior replacements. The scope of work includes: * Replacement of front and back doors with new locks. * Replacement of flooring with luxury vinyl tile. * Replacement of all cabinets in the kitchen, restrooms, and utility room. * Replacement of all sinks and faucets in restrooms and kitchen. * Replacement of all countertops. * Replacement of bathtubs with standing showers. * Replacement of existing propane furnaces with new propane furnaces with split AC systems. * Replacement of crawl space HVAC flexible duct. * Replacement of baseboards. * Replacement of utility room subflooring with 3/4 inch plywood. * Installation of one more layer of cinder block around each modular home and replacement of vent panels. Additionally, the Statement of Work details requirements for the installation of new office furniture in rooms 156, 157, and 158 at the Tohatchi Health Center. This includes providing and installing U-shaped desks with hutches, sit-to-stand desks, bookcases, storage cabinets, file pedestals, lateral file cabinets, and office chairs. All furniture must be wipeable with antimicrobial surfaces. The contractor is responsible for site visits to verify measurements, and all work must adhere to safety standards, housing codes, and infection control risk assessment protocols. The contractor must also dispose of debris at an approved EPA disposal location. Work hours are generally Monday through Friday, 8:00 AM to 5:00 PM, excluding Federal Holidays, though weekend or after-hours work may be necessary with prior agreement. The contractor must obtain an identification badge before starting work and wear it on premises, checking in with security daily. COVID-19 requirements must also be followed. The period of performance for the renovation is from August 1, 2026, to July 31, 2027. The period of performance for the furniture installation is from the date of award through three months to complete the installation.
