The Legacy Link, Inc., Area Agency on Aging is requesting proposals for the provision of services in the northeast Georgia region for Fiscal Year 2028 (July 1, 2027 - June 30, 2028). Services include evidence-based wellness programs (Bingocize and A Matter of Balance), health promotion/disease prevention services (nutrition education, lifestyle management, physical activity, health awareness), and senior center services in Towns and/or Union County, including congregate meals, health promotion activities, home-delivered meal staging, and transportation vouchers. Providers must be fully licensed and HIPAA compliant.
