Description
Ramsey County seeks agencies to provide case management for residents in Alternative Care, Brain Injury, CADI, Community Alternative Care, Developmental Disability, Elderly Waiver, Essential Community Supports/Rule 185, and Vulnerable Adults/Developmental Disability Targeted Case Management programs. Services are voluntary and must use person-centered practices. Providers will help residents access waiver and other needed services, monitor plans of care, maintain compliant case-management records, and provide culturally responsive programming. Contractors are expected to accept referrals based on county need and participate in at least two community events annually. Responses are evaluated on qualifications and experience, work plan, racial equity and inclusion, and community engagement. The solicitation anticipates multiple awards and a contract term from January 2027 through December 2031. Payment is based on Minnesota Department of Human Services rates; the solicitation lists current program rates. Responders must meet applicable state qualification standards and have current or recent waiver case-management experience with a lead agency or tribe.
