Description
The Town of Flower Mound is seeking statements of qualifications from qualified firms to establish a two-year list of approved providers for professional material-testing services for various projects. Services may support roadway, utility, trail, and other construction work. Selected firms will provide project-specific proposals based on approved unit costs and perform testing in accordance with applicable standards. The response must be submitted electronically in PDF format and include the Town of Flower Mound Proposal Certification Form, a completed and signed Conflict of Interest Questionnaire, and a current Certificate of Insurance. Respondents must also provide qualifications and relevant project experience, references, business history, quality-assurance information, project-manager and key-personnel information, team roles, proposed services and capacity, and disclosures of significant contract failures, pending litigation, or investigations.
