Description
The Louisiana Department of Health, Office of Aging and Adult Services, is seeking information from potential providers to inform the development and expansion of Programs of All-Inclusive Care for the Elderly (PACE) in Louisiana areas not currently served by a PACE organization. Responders must describe their proposed PACE center locations, service areas, number of centers, opening timelines, rural service strategies, capacity to support nursing-facility residents transitioning to community living—including eligible individuals with serious mental illness—and approaches to affordable housing. PACE providers are expected to deliver and coordinate comprehensive Medicare- and Medicaid-covered services, including adult day health care, primary and specialty medical care, behavioral health, home health and personal care, therapies, diagnostics, transportation, medications, social services, durable medical equipment, respite, hospital care, and nursing-facility care. Optional information may include organizational history, partnerships, financial statements and projections, marketing and outreach, startup costs, existing PACE operations, location-specific challenges, transition resources, and affordable-housing experience. This is an RFI for planning purposes only and is not a solicitation or commitment to award a contract.
