Description
The Ohio Department of Rehabilitation and Correction, through Ohio Penal Industries, is soliciting bids for biodegradable HD/LLDPE polyethylene tubing used to manufacture plastic bags. The required materials include film thicknesses from 0.236 to 0.9 mil, widths from 24 to 44 inches, specified roll widths, star seals, and black, translucent, or various-color options. The item grid lists 900,000 linear feet for each required material specification. Bidders must state delivery time after receipt of order and must be able to provide all requested materials within 30 days after receipt of order, unless otherwise mutually agreed with the agency. The response must address material warranty information, compliance with agency policies and contracting terms, and the supplier's ability to fulfill the invitation-to-bid requirements. The questionnaire also requests an affirmation and disclosure form, insurance certificate, Ohio registration documentation, and Ohio workers' compensation documentation.
