Description
The Utah Cooperative for Acquiring Resources Efficiently (UCARE), with Canyons School District managing the procurement, seeks suppliers to process USDA commodity mozzarella cheese into pizza products for participating school districts. Suppliers must be USDA-approved processing plants and approved by the Utah State Board of Education Child Nutrition office. Products must meet or exceed the listed specifications, contain zero trans fat, and be supported by current Summary End Product Data Schedules, CN labels, and/or product specification sheets. The price schedule lists multiple pizza products, including cheese, pepperoni, turkey pepperoni, and specialty varieties; suppliers must provide pricing for the specified delivery and processing options. Proposals must include the required schedules and certifications, food-safety violation history, evidence of a processing-facility audit score of at least 90%, current signed SEPDS information, pricing and value pass-through details, and a food recall plan with primary and backup contacts. References are required, and product samples may be requested after proposal closing for competitive items. The contract is firm-fixed-price for one year, with up to four annual extension options. Estimated usage is approximately 29,153 pounds for the stated fiscal year, but this is not guaranteed.
