Description
Gilbert Public Schools is soliciting a complete group short-term disability insurance program covering both employer-paid and voluntary employee-paid plans. The selected vendor must provide at least the existing benefit levels, administer eligibility and enrollment, accept electronic eligibility files, process claims and benefit payments, coordinate benefits, handle denied-claim reviews, provide customer service and account management, support open enrollment, furnish bilingual educational materials and policy or certificate booklets, and provide reporting and secure online access. The employer-paid plan covers approximately 3,572 employees and the voluntary plan approximately 1,095 employees. The requested employer-paid design includes 90-day accident and sickness elimination periods, benefits of up to 66 2/3% or 75% depending on class, a 13-week benefit period, and a $750 maximum weekly benefit for Class 1. The voluntary design includes 14-day accident and sickness elimination periods, benefits up to 66 2/3%, an 11-week benefit period, and a $1,000 maximum weekly benefit. The initial performance period is July 1, 2027, through June 30, 2028, with a rate guarantee through June 30, 2030 and options for two additional one-year terms.
