Description
The State of Hawaii Department of Health, Executive Office on Aging, is requesting information from providers and community stakeholders to inform development of a forthcoming KupunAloha pilot program. The anticipated program would provide coordinated, technology-enabled in-home health care and support services on Oahu for elderly individuals who do not qualify for Medicaid or other publicly funded long-term services and supports and cannot afford private care. The proposed model would integrate licensed paid caregivers, volunteers, family caregivers, care navigators, licensed in-home health care agencies, and a secure HIPAA-compliant technology platform. Responses may address program design and feasibility, technology capabilities, volunteer engagement, licensed-agency participation, intake and eligibility, care navigation, payment models, budget and sustainability, workforce challenges, and Hawaii in-home-care rates. Respondents should provide organization and contact information, identify their organization type, answer any relevant questions, and may include supporting materials. Participation is optional and does not obligate the State to issue an RFP or award a contract; if the State receives the anticipated federal grant, the RFI states that selected providers named in the grant application may be awarded a contract.
