Description
Sarasota County is seeking a licensed physician to provide Medical Director services for Sarasota County Fire Department Emergency Medical Services in compliance with Section 401.265, Florida Statutes. The anticipated initial contract term is five years. The solicitation requires a written proposal covering qualifications, experience, approach, and compensation, along with a completed Vendor Information Form, Reference Form, Public Entity Crime Statement, and Foreign Country of Concern Attestation Form (PUR 1355). A Local and American Business Certification Form is optional for proposers seeking the applicable preference, and a Joint Venture Form is required only for joint ventures or partnerships. The written proposal must be uploaded as a single readable document, and references must be submitted using the County-provided form.
