Description
The City of Omaha is soliciting proposals for a healthcare provider to establish and operate an annual NFPA 1582-focused wellness program for Omaha Fire Department personnel. The selected provider must process up to 690 physical examinations and cancer screenings within 180 days, serve up to eight individuals per day, and perform examinations, procedures, consultations, and sample testing at one location during one visit. The facility must be a Nebraska-based Accountable Care Organization located within the Omaha Fire Department response territory. Required services include medical-history and behavioral-health questionnaires; comprehensive physical examinations; pulmonary-function testing; annual resting ECGs; rotating biennial cardiac stress testing and electron-beam tomography heart scans; blood and urine laboratory testing; cancer screenings; physician consultations and referrals; secure handling of medical information; and anonymous aggregate health-trend reporting. Optional separately priced services include vision testing, audiograms, chest radiographs, immunizations, and infectious-disease screening. Proposals must include an executive summary, corporate overview, technical approach, separate cost proposal, at least three professional references, and required compliance and bid-security materials. The cost proposal must be uploaded separately because no pricing lines are provided in the bid event. The bid bond surety must be licensed by Nebraska and listed in the current U.S. Treasury Circular 570; letters of credit are not acceptable substitutes.
