The Iowa Department of Health and Human Services is soliciting applications from qualified organizations to serve as cardiovascular health Hub and Spoke sites in rural Iowa. Proposed Networks must sustain or expand access to cardiovascular diagnostics, interventional procedures, disease management, rehabilitation, chronic disease prevention, and tele-cardiology, with care delivered as close to patients' homes as clinically appropriate. Applicants may propose one of three options: a regional Hub and Spoke Network for advanced cardiovascular services; expansion of a single organization's capacity to become a regional cardiovascular Hub; or a network supporting primary and secondary cardiovascular care through prevention, chronic disease management, diagnostics, outpatient or lower-level therapeutics, rehabilitation, and virtual consultations. The program is supported by a potential five-year financial assistance award, with activities and funding subject to approval of the agency's year-two budget.
