The State of Tennessee, through the Department of Children's Services, is seeking a single provider to deliver statewide Bright Futures case management services for young adults ages 18–23 who have aged out of foster care or exited foster care through adoption or subsidized permanent guardianship after age 16. Services include transition planning, face-to-face contacts at least every 30 days, home visits, eligibility verification, life-skills and needs assessments, education and employment support, health-care advocacy, assistance obtaining essential documents and benefits, court and review-board coordination, electronic case documentation, after-hours support, and post-discharge outcome surveys. The response must address mandatory qualifications, organizational and staffing qualifications, technical approach, statewide service capacity, information-security requirements, and references for two current and three completed comparable contracts. The contract is a single award with compensation of $26 per day per client; the Release #2 pro forma contract states a maximum liability of $51,246,000 and a term ending June 30, 2029, with the effective date left blank. Responses must not include cost information in the technical response.
