Description
Bergen County is soliciting qualification statements from firms or organizations capable of providing on-site physical therapy services for inmates at the Bergen County Jail. The selected provider must have a current New Jersey physical therapy license, a Doctorate in Physical Therapy, multi-year experience providing on-site physical therapy in a county jail, participation in an orthopedic hospital rehabilitation network, and the ability to provide on-call service and on-site service an average of twice per week. Respondents must submit a qualification statement, professional experience and resumes, references, a narrative addressing the County’s needs, a fee schedule for single-patient, two-patient, and three-or-more-patient visits, and a charge for initial evaluations. The submission must also include required business, ownership, tax, licensing, insurance, affirmative-action, and statutory disclosure documents. The service contract will be valid for one year on or about the date of award.
