Description
The Texas Workforce Commission is conducting an open enrollment for diabetes self-management education services for Vocational Rehabilitation customers. Services must develop customers’ knowledge, skills, and abilities to manage diabetes and improve health outcomes, including adaptive techniques and equipment for customers who are blind or visually impaired. The scope includes close, hands-on assessment and training in blood sugar monitoring, medication delivery, and other diabetes self-care skills. Services are based on the Association of Diabetes Care and Education Specialist’s seven self-care behaviors: healthy eating, physical activity, monitoring, medication use, healthy coping, problem-solving, and risk reduction. Eligible customers may be newly diagnosed, at risk of complications, unable to maintain employment because of diabetes, in need of additional training after vision loss, unable to benefit from community-based programs, or have needs not met by standard medical providers.
