Description
The Maryland Department of Health's Office of Long-Term Services and Supports is soliciting up to eight awards from licensed Medical Adult Day Care Centers to provide subsidized adult day care services to functionally impaired Maryland residents aged 55 and older who are at risk of institutional placement. Services include a minimum four-hour actual day of service, compliance with applicable Maryland regulations, participant attendance and health records, fee assessment and billing support, and staffing by a program director, licensed registered nurse, activities coordinator, licensed medical director, and licensed social worker. Contractors must serve designated zip-code areas in eight Maryland counties, provide transportation to and from the center for current participants who request it, maintain required participant records and reports, and accept payment based on the State's ability-to-pay schedule. The procurement is an indefinite-quantity, firm-fixed-unit-price contract with a three-year base period and two one-year options. Bids are evaluated by daily rate and geographic location, with a 5% MBE subcontracting goal.
