Description
The Department of Veterans Affairs is seeking capability information from qualified providers able to furnish community-based transitional housing and medical respite or recuperative care for homeless Veterans who do not require hospital-level care. Services must include 24/7 accessible housing, meals and snacks, laundry and hygiene supplies, case management, therapeutic and structured group activities, medication monitoring, recovery support, transportation, assistance with benefits and permanent housing, healthcare coordination, and accommodation of visiting skilled home-health providers. Facilities must meet specified licensing, occupancy, safety, accessibility, sanitation, fire-protection, medical-equipment, privacy, communications, backup-power, and transportation-access requirements. Providers must maintain qualified 24/7 staffing, including case-management and medical personnel, background checks, required training, documentation, incident reporting, inspections, and compliance with VA program standards. This is a Sources Sought notice for market research only, not a solicitation or request for proposals. Respondents must submit a capability statement and company, NAICS, UEI, business-size, socioeconomic, and relevant capability information.
