Description
The Aging and Long-Term Services Department is accepting applications from senior centers for capital outlay funding for meal-delivery, passenger, accessible, or specialized vehicles needed for senior-center services. The application requires a completed vehicle-and-equipment questionnaire, certification letter, ICIP document, scope-of-work and deliverables worksheet, project budget and funding-sources worksheet, applicable quotes or cost estimates, an ongoing maintenance budget, and a fleet maintenance and management plan. The attached worksheets require project information, requested and total project costs, funding sources, advance-funding confirmation, work activities, deliverables, milestones, responsible parties, and authorized certifications. Required files must be submitted as PDFs unless otherwise specified; the vehicle-and-equipment questionnaire uses an Excel response template.
