Description
The City of O'Fallon is soliciting a transportation provider to serve elderly, disabled, and low- to moderate-income residents who reside in O'Fallon, Missouri. Required services include curb-to-curb ambulatory and paralift transportation within St. Charles County, round-trip rides for necessary medical appointments, home-to-door assistance, and transportation for life-sustaining appointments during holidays, public health emergencies, and disaster declarations. The provider must also handle program administration, detailed billing, monthly invoices and usage reports, complaint tracking, and participant records. The initial contract term is one year, with four optional one-year renewals. Bidders must provide pricing for one-way trips, mileage-based trips, and administrative costs for the initial and renewal years, and must submit the completed bid forms and required certifications. The solicitation requires an original certificate of insurance and specifies commercial general liability and automobile liability limits of $3,500,000, with workers' compensation coverage required.
