Description
The Department of Veterans Affairs is soliciting a firm-fixed-price, indefinite-delivery/indefinite-quantity contract for transitional housing services for up to 30 homeless male Veterans under the Low Demand Safe Haven model. The contractor must provide a safe, secure, 24-hour-staffed residential environment; same-day admissions; meals and snacks; linens; laundry access; hygiene supplies; transportation; janitorial and housekeeping services; medication management; security; and a safe room. The contractor must also provide outreach, case management, therapeutic and rehabilitative group activities, permanent-housing search assistance, financial and employment services, discharge planning, and coordination with VA and community providers. The contractor must maintain sufficient staffing, including a program manager, licensed clinical supervisor, case managers, monitors, security staff, and housekeeping staff. Services include daily census and bed checks, incident reporting, HMIS participation, billing documentation, quality-control activities, inspections, staff training, and compliance with accessibility, privacy, security, life-safety, and applicable licensing requirements. The contract has a five-year period of performance, with a guaranteed minimum of $1,000 and a maximum aggregate ordering value of $11,000,000. The solicitation anticipates one or more awards and requires complete pricing for five annual ordering periods.
