Description
The Colorado Department of Human Services is seeking qualified healthcare organizations operating skilled nursing facilities to establish and operate a minimum of 12 enhanced protective placement beds, with potential expansion based on demand and funding. The program will serve individuals with severe mental illness, brain injuries, advanced dementia, acquired neurocognitive disorders, and complex behavioral or medical-behavioral needs, including people transitioning from state hospitals or requiring highly supported community placement. The contractor must provide both secure memory care and unsecure residential settings, 24/7 onsite skilled nursing and clinical support, individualized therapeutic, behavioral, rehabilitative, physical, occupational, and speech services, specialized neurocognitive programming, behavioral de-escalation, medication stewardship, and multidisciplinary clinical oversight. The facility must maintain applicable nursing-home and behavioral-service licensure, support admissions and referrals coordinated by the Department, and be operational by October 15, 2026, or the earliest possible date following contract execution. Compensation includes a $307.64 per-bed-per-day availability incentive through June 30, 2027, capped at $3,691.34 per day for 12 beds, plus up to $45,920 in documented capital and renovation reimbursement.
