The Wayne County Board of Developmental Disabilities is seeking an agency provider to deliver Home and Community-Based Services (HPC) for three individuals with IO Waiver funding residing in a congregate setting in Wooster. The home requires 24/7 staffing with overnight OSOC support. Residents are ambulatory and fairly independent in most activities of daily living with prompting and encouragement. Responsibilities include managing appointments and medical issues (Med Certification 3 required), transportation to appointments, money management support, meal planning and preparation, communication with guardians and teams, and general supervision.
