Description
The City of Cleveland is requesting 1,000 personalized, clear-blue, 18-ounce plastic water bottles with push caps and a city emblem. The logo must be placed near the top of each bottle, printed in white, with “Community Relations Board” printed directly beneath it and the logo and wording spaced evenly. Pricing must include delivery to the designated destination on a free-on-board delivered basis, with shipping charges included in the quoted unit price. The bid package must include the completed request-for-bid form, terms and conditions, Northern Ireland Affidavit, and Wage Theft Form. The Northern Ireland Affidavit must be completed, signed, and included with the bid; the Wage Theft Form requires disclosure of applicable wage-theft or payroll-fraud determinations.
