Description
Adams County is establishing a pool of qualified providers for services to children and families involved in the child welfare system. Requested service categories include family time and visitation, home-based intervention, intensive family therapy, life skills, mental health services, sexual abuse treatment, substance abuse treatment, and child mentoring and support. Services are intended to strengthen families, protect children, prevent out-of-home placement, and support reunification and permanency. Applicants must submit a fee schedule for each service type proposed; the County may select providers from the pool for project-specific agreements and referrals. Providers must meet applicable staff qualification and background-check requirements. The application requests information about agency experience, service delivery, staffing and availability, outcomes, collaboration, reporting, and Medicaid participation. Applicants must provide proof of active SAM registration, a certificate of insurance, a W-9, and the required fee schedule. The sample agreement is informational and is not a submission form.
