Description
The Minnesota Department of Human Services, through its Health Care Administration and Office of the Medicaid Medical Director, seeks up to three vendors to analyze access to whole-person perinatal care and care coordination for the Transforming Maternal Health Model in Hennepin County. Responders may propose work for one, two, or all three priority areas, and the State may make one or multiple awards. The total amount available is $300,000, approximately $100,000 per priority area. The work includes comprehensive gap analyses, process and journey mapping, community and provider engagement, and actionable policy and operational recommendations. Priority Area 1 addresses screening, referrals, and follow-up for perinatal depression, anxiety, substance use, and tobacco use; Priority Area 2 addresses screening, referrals, and follow-up for upstream drivers of health, including food, housing, transportation, and intimate partner violence; and Priority Area 3 addresses telehealth and home monitoring for gestational diabetes and hypertension, including related devices, applications, billing, and follow-up. Required deliverables include gap-analysis reports, process and journey maps, presentations, one-page summaries, engagement sessions and feedback analysis, implementation-plan recommendations, workflow and referral diagrams, Medicaid billing and reimbursement analysis, monthly progress reports, invoices, meetings, and support for evaluation and federal reporting.
