Description
The State of Nevada Purchasing Division is seeking statements of qualifications from providers of behavioral and community-based services, medical services, and job development services. Multiple contracts may be awarded for statewide or regional service coverage, including Northern, Southern, and Rural regions, with no guaranteed minimum service volume. The resulting contracts may be used by state agencies and, at their discretion, other listed public entities. Providers must submit a completed Provider Application Response, identify the service areas and geographic regions they can support, address mandatory qualifications and critical items, and upload the response through NevadaEPro. Requirements include current Nevada professional licensure where applicable, a Nevada business license, State Controller registration, disclosure of significant contract failures or litigation, agreement to the included contract form, and applicable insurance information. The initial contract term is four years, with rolling enrollment continuing quarterly; the State may close the solicitation at any time.
