Description
The City of Phoenix Retirement Office, on behalf of the Phoenix Fire Pension Board and Phoenix Police Pension Board, seeks professional services for disability medical evaluations. The contractor will review and chronologically summarize applicants’ medical records, identify appropriate medical specialties, maintain qualified physician panels, schedule examinations, coordinate communications, and provide related administrative support. The scope also includes signed medical evaluation reports, supplemental reports, specialized testing, occasional on-site or records-only evaluations, rescheduling and no-show support, and rare expert testimony. Examining physicians must be regularly practicing Arizona-licensed M.D.s or D.O.s with relevant experience, including experience performing independent medical evaluations for Arizona Public Safety Personnel Retirement System members. The contract has a maximum authorized amount of $750,000 and is structured as a three-year base term with two one-year options.
