Description
The State of Maine Department of Health and Human Services, through its Office of Aging and Disability Services, seeks one contractor to support people with acquired brain injuries and their families. Required services include a statewide telephone hotline operating at least from 8:00 a.m. to 4:45 p.m. Eastern Time, information and referral, community resource coordination, neuro-resource facilitation, transition and ongoing support, hospital outreach and discharge-planning clinics, and linkage to existing services. The contractor must also train and support peer and family support groups, provide monthly statewide brain-injury training, coordinate Maine ABI Advisory Council meetings, participate in provider meetings, and plan and host an annual Brain Injury and Family Caregiver Conference using a hybrid in-person and virtual format for at least 100 participants. The contractor must use secure HIPAA-compliant technology where applicable, meet specified performance measures, and submit recurring operational, training, program, financial, and closeout reports. Proposals must include organizational qualifications, three recent relevant projects, staffing and work plans, financial statements, proposed services, and a cost proposal.
