The Department of Veterans Affairs is seeking brand-name-or-equal replacement dental equipment for the St. Louis VA Health Care System. The requirement includes nine A-dec 511 chairs, eight A-dec 532 TRAD delivery systems, nine A-dec 482 monitor mounts, nine A-dec 521 doctor stools, three A-dec 522 assistant stools, one Unit MT Module 300PRO on 511B, and installation. The equipment must meet specified seat-height, hydraulic lift and tilt, weight-capacity, electrical, and handpiece-compatibility requirements. The contractor must provide delivery, blanket-wrap protection, transportation to final-use locations, de-installation and removal, assembly, installation, utility connections, testing, debris removal, on-site clinical and biomedical-engineering training, manuals, and at least a one-year manufacturer warranty covering parts and labor. Delivery is required within 75 business days after receipt of order, with installation completed within 60 days after delivery. The quote package must contain three separately labeled PDF attachments covering the signed solicitation and required certifications, technical capability, and pricing; an OEM authorization letter and applicable product certifications are required.
