Description
The West Virginia Department of Health, Bureau for Public Health, Office of Maternal, Child and Family Health, is offering funding for a statewide parent-led, patient-centered medical home education program for children with special health care needs. The selected grantee will adapt and expand the Project DOCC curriculum, recruit and train parents as host and visiting teachers, coordinate home visits and parent interviews, and provide instruction to medical students, residents, and physicians on family-centered care, community resources, and medical-home concepts. The scope also includes resource navigation and one-on-one family consultations, parent-to-parent support, community and conference participation, maintenance of a resource-focused social media presence, development and distribution of brochures, collaboration with pediatric physicians and medical trainees at Marshall University, and monthly and annual performance and financial reporting. Maximum available funding is $82,453. Eligible applicants are nonprofit organizations or coalitions licensed to operate as businesses in West Virginia and registered state vendors. Applications must include a proposal narrative, detailed line-item budget and budget narrative, Subrecipient Information Form DHHR Finance A-1000, and a grantee contact list, and must be submitted through the WVOASIS Vendor Self-Service Portal.
