Description
Louisville Metro Public Health and Wellness is seeking a contractor to provide comprehensive clinical and support services for Healthy Start participants. The scope includes evidence-based individual and group behavioral health counseling, therapy, screenings, assessments, stress-management interventions, perinatal mental health education, crisis support, referrals, and care coordination. The contractor must also provide licensed Certified Nurse Midwife services, including prenatal and postpartum consultations and education, reproductive health counseling, health assessments, risk-reduction activities, childbirth preparation, breastfeeding and family-planning education, and coordination with medical providers and community resources. Additional responsibilities include participant outreach and retention, transportation-related support, incentives, flexible service delivery, multidisciplinary case discussions, and culturally responsive, trauma-informed group programming. Proposals are limited to 40 pages and must include qualifications, a technical proposal, experience examples with references, and a separate cost proposal, along with the required vendor, acknowledgment, compliance, and good-faith-effort materials.
